Applicants must provide all requested documentation including picture identification for all members of the household who will be receiving benefits, proof of address and proof of income. The amount of monthly assistance provided is determined by several factors, such as monthly income and household size. Recipients of the program are required to work or attend school for a minimum of 30 hours per week unless they can show acceptable proof that they are unable to do so.
